To try this feature, edit the Model Driven App in new modern designer
Once the designer is opened, you can select the entity you want to customize and click on Edit Command bar
You will be redirected to below screen
you can select which command bar you want to customize, I am customizing form so selected Main Form option and click on edit you will redirect to below screen
click on New to add new button
Provide details like Label, Tool tip , Icon . You can use existing icons or import your own icon using web resource
For the button, on select property you can either write the Fx formula or call the JavaScript function
Button visibility can also be handled through visibility action
Features:

Open a custom page as a dialog
You can also trigger cloud flows from a button on the custom page
